Synergy BJJ MMA Singapore Membership Contract

This contract covers the purchase of a membership at Synergy BJJ MMA Singapore.
NOTICE OF DUES RENEWAL: This membership agreement will renew automatically at the end of each contract’s period, with monthly dues to be paid on the activation date of each month. If at the end of the annual term member does not wish to renew, the member will have two weeks in which to cancel the renewal without penalties. Synergy BJJ MMA Singapore reserves the right to change the amount of dues and charges payable hereunder at any time after the end of the initial term. Members may cancel this agreement at any time within the initial term by written notice to Synergy BJJ MMA Singapore only for reasons complying with the terms below.
CANCELLATION PROCEDURES:
1.If a member dies or becomes totally or permanently disabled after the date this contract takes effect, the member or member’s estate may cancel this contract and receive a partial refund of unused membership fee. Proof of death or disability will be required.
2.If this gym closes and does not have another location within 32 km, members may cancel this contract by sending a letter by certified mail to the corporate office. If a member moves more than 100 km away from the gym, this contract may be canceled with proof of new address and phone number. There will be no other right to cancel, including for non-usage of facility.
FREEZING
- Temporary Illness or Injury: This agreement may be frozen in the event of a temporary illness, injury in our classes or medical condition which in the written opinion of a doctor or other suitably qualified medical practitioner prohibits exercise for a period of time.
- Temporary overseas for a length of time: This agreement may be frozen in the event of a business trip for a length of at least 14 days.
Please note – ANY Freeze will not be affected until the appropriate proof is provided and received (in writing or via email) by Synergy BJJ MMA Singapore.
PAYMENT DEFAULT: If a member fails to pay any amount when due under this agreement, Synergy BJJ MMA Singapore shall be entitled to suspend or cancel the member’s membership and to require the member to immediately pay all past due balances. Suspension or cancellation shall not relieve members from the obligation to pay any unpaid balances.
By purchasing a membership from Synergy BJJ MMA Singapore, you agree to be bound by the following terms as above for the duration of the term of your membership.
